Who Can Receive Our Services

To qualify for community nursing services, you must have

A Veteran Gold Card, or A Veteran White Card

If you have a Veteran White Card, services are funded only if related to service-related injuries or conditions. Please check with DVA to confirm funding before starting care.

Referring a Client

When to Refer:

A written referral is needed if a client is:

  • New to community nursing services
  • Returning after more than 28 days
  • At the end of each 12-month care period

 

Once we receive a referral, we will:

  • Assess the client’s needs
  • Provide care according to the CN Program’s guidelines

  • A general practitioner (GP)
