Who Can Receive Our Services
To qualify for community nursing services, you must have
A Veteran Gold Card, or A Veteran White Card
If you have a Veteran White Card, services are funded only if related to service-related injuries or conditions. Please check with DVA to confirm funding before starting care.
Referring a Client
When to Refer:
A written referral is needed if a client is:
- New to community nursing services
- Returning after more than 28 days
- At the end of each 12-month care period
Once we receive a referral, we will:
- Assess the client’s needs
- Provide care according to the CN Program’s guidelines
Who Can Refer:
Referrals must come from:
- A general practitioner (GP)
- A hospital doctor
- A hospital discharge planner
- A nurse practitioner specializing in community nursing
